Justin Smith was born on September 30, 1979 in Jefferson City, Missouri, and grew up on cattle ranches around Holts Summit. He played defensive end at Missouri, and Cincinnati drafted him fourth overall in 2001. He started eleven of fifteen games as a rookie, and Madden NFL 02 rated him 79 for that season. He started every game of the next six years for the Bengals, and Madden NFL 03 rated him 89 for 2002.

San Francisco signed him in 2008 and used him inside a three-man front. He made five straight Pro Bowls from 2009 through 2013 and was a first-team All-Pro in 2011 and 2012. Madden NFL 12 rated him 92 for the 2011 season. He tore his left triceps against New England on December 16, 2012, which ended a run of 185 consecutive starts. He returned for the playoffs and played in the Super Bowl XLVII loss to Baltimore, and Madden NFL 13 rated him 98 for that 2012 season.

A left shoulder injury that began in 2013 training camp troubled him through his final two years, though he started all sixteen games in each. Madden NFL 15 rated him 92 for 2014. He announced his retirement in May 2015 after fourteen seasons and returned to Missouri. He finished with 221 games played, 884 tackles and 87 sacks.
