Alex Smith was born on May 7, 1984 in Bremerton, Washington, and played college football at Utah, which went 12-0 in his final season. San Francisco drafted him first overall in 2005, and Madden NFL 06 listed him at 82 for that season. He cycled through offensive coordinators, lost the 2008 season to a shoulder injury, and Madden NFL 10 dropped him to 67 for 2009.

Under Jim Harbaugh he started all 16 games in 2011 and reached the NFC Championship Game. A concussion in November 2012 cost him the job to Colin Kaepernick, and San Francisco traded him to Kansas City in March 2013. He was selected to Pro Bowls in 2013, 2016 and 2017, and Madden NFL 15 gave him his highest mark, 89, for 2014.

Washington acquired him in January 2018. On November 18, 2018, against Houston, he broke the tibia and fibula in his right leg; infection and sepsis followed, and he underwent 17 surgeries. He returned in 2020, appeared in eight games with six starts, and won the Associated Press Comeback Player of the Year award. Madden NFL 21 listed him at 68 for that season. He retired in April 2021 with 35,650 passing yards and 199 touchdowns.
