The blood-clot scare that made Kansas City a sixth-round fortune
Trey Smith was ESPN's number one overall recruit in the 2017 class, a two-time first-team All-SEC guard at Tennessee. Then blood clots in his lungs, first found in early 2018 and recurring that October, cost him a season and scared off the entire first day of the 2021 draft. Kansas City grabbed him in the sixth round, 226th overall, and got a starter from day one.
Smith made the All-Rookie team and started every game through four seasons, moving people at the point of attack for a Chiefs front that won Super Bowl LVII over Philadelphia and Super Bowl LVIII over San Francisco. In July 2025 he cashed in fully, signing a four-year, $94 million extension with $70 million guaranteed, the richest deal for a guard in the league.
Madden's read on him is a straight climb from the medical red flag to elite. He entered at a 63, jumped to 82 by 2023, and settled in at a 90 across both the 2024 and 2025 editions.
